Getting started is easy!

We understand that you might be apprehensive about your first physical therapy appointment but as soon as you set foot in one of our clinics, you'll feel like you belong to APT. Our welcoming front office staff is happy to help you begin your path to recover and answer any insurance, scheduling, or billing questions along the way.

When should I arrive?

Please arrive 15 minutes before your scheduled appointment time in order to complete all necessary paperwork. We advise that you also download and complete the forms ahead of time to save time and get started quicker. The necessary new patient forms are listed below.

Initial Evaluation


The initial evaluation is a comprehensive examination of your particular ailment by a licensed therapist and is the first step in the rehabilitation process. This plan typically consists of attending 2-3 therapy sessions per week for direct supervision and progression of exercise and may also include therapeutic modalities, massage, and manual therapy. 

Insurances Accepted

Action Physical Therapy accepts most insurance plans including Medicare. However, please call our billing office at (405) 214-0300 in advance of your first visit so that may verify our participation with your particular health plan and insurance benefits.
